Position Description

RN – Case Manager

Facility: Santa Fe Service Unit (PHS Indian Hospital)

Location: Santa Fe, NM 87505

Assignment Length: 32 Weeks

Shift: 8:00 AM – 4:00 PM

Guaranteed Hours: 40 hours/week

Compensation Package

Hourly Rate: $60/hr
Weekly Gross (40 hrs): $2,400
Holiday Rate: Flat $60/hr (same as regular rate)

Position Requirements

Active Registered Nurse (RN) license
3 years of professional nursing experience
At least 1 year of Case Management/Discharge Planning experience
Must submit all current certifications at the time of application

Key Responsibilities

Coordinate and manage patient care plans, ensuring safe and timely discharge planning
Collaborate with physicians, healthcare providers, and families to optimize care transitions
Provide patient education, advocacy, and community resource referrals
Maintain compliance with facility policies and federal healthcare regulations

Additional Details

Facility is located in a remote area on the reservation, with limited housing options. Candidates should confirm housing arrangements before submittal.
